Health & Wellbeing Coaching and Community Health & Wellbeing Workers Service for Bexley

NHS South East London Integrated Care Board is commissioning a Health & Wellbeing Coaching and Social Prescribing Service for the London Borough of Bexley. This service supports residents aged 18+ living in or registered with a Bexley GP practice to manage long-term conditions and improve health outcomes through coaching, peer support, and community connections. The service comprises two components: (1) 2 full-time Health & Wellbeing Coaches providing community outreach across three neighbourhoods (Frognal, Clocktower, and North Bexley) for 3 years, and (2) 5 Community Health & Wellbeing Workers based in North Bexley focusing on the Slade Green and Erith areas. The provider must be an established voluntary sector organization already delivering social prescribing and peer support services in Bexley, with proven integration into neighbourhood health teams. Key responsibilities include identifying eligible residents, conducting health checks, delivering motivational coaching, signposting to community resources, and supporting residents toward self-management of conditions. The service addresses health inequalities in Bexley, where approximately 26,000 residents have multiple long-term conditions and deprivation-related life expectancy gaps of 2-5 years. Successful applicants must mobilize within 3 months of contract award, with potential for service expansion dependent on future funding. This represents an opportunity to expand services addressing wider determinants of health and prevent escalation into crisis care.
