Surgical Implants Supply for Breast Surgery

This contract involves the supply of medical products used in breast surgery for Copernicus Medical Entity in Gdańsk, Poland. The detailed description, assortment, and estimated quantities are provided in the pricing and assortment form (Attachment No. 1 to the SWZ, parts 1-3). The offered products must be complete, of high quality and functionality, and free from material, construction, and legal defects. The products must be approved for sale and use in accordance with applicable laws. They must comply with the Medical Devices Act of April 7, 2022. Suppliers must provide technical data sheets or catalog cards from the manufacturer for the offered items. If equivalent solutions are proposed, documentation confirming compliance with the requirements must be included. Suppliers must not be excluded from the procurement process due to specific circumstances outlined in the law. A bid security of 20,000 PLN is required for total offers, with specific amounts for each part of the contract. The security must be submitted before the bid submission deadline and maintained until the offer binding period expires.
