    This is an opportunity to provide short break group activities for children and young people (aged 15-17) with Education, Health and Care Plans (EHCPs) across Tameside. The service targets young people who require one-to-one or two-to-one support due to behavioural needs, personal care requirements, safety concerns, or medical needs. Providers should use a person-centred approach, focusing on outcomes specified in individual EHCPs and promoting independence where possible. Age groups should be organised thoughtfully, avoiding mixing the youngest and oldest within the age range. The service should work closely with families, the Children with Disabilities team, and partner agencies. For young people aged 17, providers must support transition planning into adult services in collaboration with relevant partners. Staffing should be flexible and responsive, with the ability to reduce support ratios when safe to do so and agreed with commissioners and the Children with Disabilities team.
