Extracare and Supported Living Services for Older Adults and Vulnerable Persons in Cardiff

Cardiff Council is seeking qualified service providers to deliver integrated extracare and supported living services for older people (aged 55+) and vulnerable adults, including those with dementia, cognitive impairment, physical/sensory impairment, or mobility issues. Services will be provided across five schemes (Llys Enfys, Plas Bryn, Llys Y Goetre, Dol-Yr-Hafren, and Cwrt Hir) totaling approximately 240 self-contained flats and bungalows. The successful provider will deliver personalized, outcome-focused care and support that promotes independence and inclusion, with on-site teams available 24/7/365. Day-hour support runs 7am-11pm assisting with personal care and wellbeing, while night provision includes waking night staff (11pm-7am) and sleep-in arrangements as needed. Services must comply with the Regulation and Inspection of Social Care (Wales) Act 2016 and support complex needs including post-hospital recovery, respite care, and reablement. The provider will work collaboratively with health services, social work teams, and community networks to deliver preventative, holistic outcomes while maintaining flexibility to respond to emergencies and hospital discharge needs.
