Stroke Recovery Support Service

This service provides comprehensive support for stroke survivors and their families across health and social care. The provider will deliver high-quality, non-discriminatory care tailored to individual needs. Key responsibilities include: helping stroke survivors maintain health and wellbeing while living independently at home and in their community; offering timely information, emotional support, practical advice, and advocacy to stroke survivors, families, and carers; and educating stroke survivors and their support networks about stroke effects and recovery. The service aims to bridge gaps between multiple care providers, ensure equitable access to support, and empower stroke survivors to achieve the best possible recovery outcomes.
