This is a direct award contract to deliver integrated health and social care services for vulnerable residents facing multiple overlapping challenges. The service combines clinical expertise with social care coordination to address immediate healthcare needs and broader factors affecting health, such as housing, employment, and financial stability. Key service elements include outreach, relationship-building, and person-centred support to help individuals re-engage with mainstream healthcare and social services. The target cohort includes people experiencing drug and alcohol dependence, mental health conditions, homelessness or housing insecurity, social exclusion, financial hardship, safeguarding concerns, and poor engagement with primary care. The service aims to fill gaps in current provision, improve system integration, and deliver better health outcomes for some of the borough's most vulnerable residents. Contract Value: £73,000.
