Consultant to Develop Hospital Discharge Support Procedure for Unpaid Carers

The South East London Integrated Care System (SELICS) is seeking a consultant to develop and implement a Standard Operating Procedure (SOP) for identifying and supporting unpaid carers at the point of hospital discharge across six South East London boroughs. The successful provider will work with healthcare and social care stakeholders to create a framework that helps unpaid carers access relevant information, support, advice, and guidance. The primary objectives are to: enable safe and timely patient discharge to home settings, reduce unnecessary referrals to statutory social care services, and prevent hospital re-admissions. This consultancy requires expertise in care coordination, stakeholder engagement, and process development to establish sustainable systems that improve outcomes for patients and their informal caregivers.
