Dementia Support Service Provider for Birmingham and Solihull

NHS Arden and Greater East Midlands CSU, on behalf of NHS Birmingham and Solihull ICB, is seeking experienced providers to deliver a comprehensive Dementia Support Service. The service will offer a single point of access for dementia information and advice, signposting to community support, emotional support, practical coaching for people with dementia and their carers, and peer-led group support facilitation. The service aims to improve quality of life through person-centered care, promote independence and wellbeing, reduce health inequalities, enhance care coordination across health and social care sectors, and prevent avoidable hospital admissions through proactive intervention. Providers must demonstrate understanding of dementia's impact on individuals, families, and communities, and establish clear referral pathways that address geographical spread and demographic needs across Birmingham and Solihull. The contract is for 2 years with an option to extend for a further 2 years. Annual contract value: £834,000.
