NHS Health Checks Service for Somerset

This is an opportunity to deliver free NHS Health Check assessments in Somerset, targeting adults aged 40-74 without diagnosed cardiovascular conditions who are registered with a Somerset GP or residing in Somerset. The service aims to reduce access barriers for people with severe mental illnesses by combining health checks with treatment access, with potential expansion to other groups such as those using bariatric services. You will deliver health checks following NHS standard protocols, including pulse rate, blood pressure, and point-of-care blood cholesterol testing. The service will identify risk factors for hypertension, diabetes, high cholesterol, kidney dysfunction, obesity, and heart arrhythmias, with findings referred to patients' GPs for formal diagnosis consideration. You will also provide lifestyle support covering alcohol use, smoking cessation, healthy weight management, and physical exercise. The service may be offered to staff or patients meeting eligibility criteria.
