Community Health and Lifestyle Services Contract

This is a 24-month contract to deliver integrated community-based health services in Peterborough, running from October 26, 2026 to October 25, 2028. The contract is procured under The Health Care Services (Provider Selection Regime) Regulations 2023. The service aims to improve population health outcomes and reduce health inequalities by providing accessible, evidence-based prevention services in community settings. Key service deliverables include: - NHS Health Checks delivered in community settings with focus on increasing uptake among eligible residents - Stop Smoking support programs with evidence-based interventions to improve quit outcomes - Smoke Free App license management and vaping product procurement and distribution in line with national guidance - Nutrition and healthy lifestyle intervention pilots - Evening and weekend appointments and workplace-based outreach clinics The provider must prioritize underserved populations including routine and manual workers, residents in social housing, and those in private rented accommodation. Services should support sustainable behavior change and specifically target cardiovascular health improvement and smoking prevalence reduction across Peterborough to address health inequalities.
