Supply of Enteral and Parenteral Nutrition Products

This contract involves the supply of enteral and parenteral nutrition products, including necessary equipment and modified milk, divided into 11 tasks. Detailed specifications can be found in Annex 1 of the SWZ, which includes the 'Assortment and Price Form.' Additional terms and conditions for the execution of the contract are outlined in 'Contract Conditions,' which is Annex 5 of the SWZ. The requirements for the pumps are also included in Annex 1 of the SWZ. Delivery is required according to current needs, within a maximum of 3 working days from the order date. The shelf life and expiration date must be indicated on the packaging and invoice. For enteral nutrition products, the shelf life must be at least half of the product's total shelf life; for parenteral nutrition products, it must be 12 months. The supplier will deliver the products to the Hospital Pharmacy Warehouse. All offered products must be authorized for sale and use in Poland and possess all required legal registration documents as per the Pharmaceutical Law of September 6, 2001. Medical products must also comply with the Medical Devices Act of April 7, 2022, and have the necessary certifications and compliance declarations. The contracting authority requires the supplier to indicate in their offer which parts of the contract will be subcontracted and to provide the names of subcontractors if known. If there is a change or resignation of a subcontractor, the supplier must demonstrate that the new subcontractor meets the same qualifications as the original. Due to the nature of the contract and changing demand, the contracting authority reserves the right to increase the order quantity by up to 50% during the contract period without requiring a contract amendment. The contracting authority will notify the supplier in writing of the need to exercise this option no later than 11 months after the contract begins. After this period, the option expires.
