Pharmaceutical Products Supply Contract

This contract involves the supply of pharmaceutical products to the Provincial Hospital in Bielsko-Biała, located at Al. Armii Krajowej 101. The products include contrast agents, hemodialysis concentrates, and sodium lime, as detailed in the quantity and type list in Annex 2 of the Terms of Reference (SWZ). The goods must be new and unused. The contract allows for equivalent pharmaceutical products, which must contain the same active substance, dosage, indications for use, and pharmaceutical form. Delivery, including transportation and unloading, will be at the contractor's cost and risk. Additionally, for specific items, the contractor must provide 5 enteral feeding pumps and 5 parenteral feeding pumps compatible with the offered products at no extra charge during the contract period.
