Medical Supplies for Mechanical Thrombectomy

This contract involves the ongoing supply of consumable materials for mechanical thrombectomy of cerebral vessels, along with the establishment of a storage facility for the Health Care Team in Bolesławiec. Suppliers must refer to the Pricing Form (Attachment No. 2) for detailed specifications. The required technical and operational parameters outlined in Attachment No. 2 are critical; failure to meet these will result in the rejection of the bid as non-compliant with the contract conditions according to Article 226, paragraph 1, point 5 of the Public Procurement Law. The contractor is required to set up the storage facility at a location specified by the buyer (Medical Warehouse) within three working days of contract signing. The first delivery must include the storage facility in all sizes requested by the buyer. The storage will be replenished after each procedure with the used set or its individual components. Partial bids are not allowed; each supplier may submit only one bid for the entire contract in written form, or it will be deemed invalid. If a supplier submits more than one bid, all bids from that supplier will be rejected. The offered goods must comply with the Medical Devices Act of April 7, 2022, and the European Parliament and Council Regulation (EU) 2017/745 of April 5, 2017, regarding medical devices, as well as amendments to Directive 2001/83/EC, Regulation (EC) No. 178/2002, and Regulation (EC) No. 1223/2009, and the repeal of Council Directives 90/385/EEC and 93/42/EEC.
