Supplies for Laboratory and Medical Waste Management

The contract involves the ongoing supply of consumable materials for laboratory testing and diagnostics, laboratory glassware, and medical waste containers for the University Clinical Hospital of Fryderyk Chopin in Rzeszów. Detailed descriptions of the required items, estimated quantities, and specifications are provided in the pricing and offer forms for Parts 1 to 12 (Attachments 1 to the SWZ), which are an integral part of the SWZ. The scope of the contract includes the delivery of the specified items in the quantities listed in the pricing and offer forms for Parts 1 to 12 (Attachments 1 to the SWZ) over a period of 24 months from the date of contract signing. The contracting authority does not allow the submission of variant or equivalent offers. The deadline for contract execution is as follows: 1) Delivery of the specified items in Parts 1 to 12 for 24 months from the date of contract signing. 2) The contracting authority may allow changes to the contract duration if the total contract items are not fulfilled during the contract period due to lower order realizations. Any changes to the contract require an annex, and the contracting authority will unilaterally notify the contractor of the proposed extension before the expiration of the contract term.
